Illness Benefit / Work Certificates
If you are sick and require a social welfare illness benefit certificate and/or a sick certificate for your employer you need to make an appointment to consult with a doctor for your first period out of work. Following this you can request repeat certificates using the form below. You may be asked to attend the doctor for review from time to time and also before you return to work.
Please be advised that your illness benefit certificate will be forwarded electronically to the Department of Social Protection. If you are requesting a work certificate for your employer, you will need to collect this from reception.
There will be a small fee for this, including for patients with medical cards and doctor visit cards. If for any reason you cannot collect your certificate we can email it to you , providing you request this at the time of ordering. We can send you a link to your phone for payment. Please be advised that this can take up to twenty four hours.
